The Mental Health Media Awards 2008 will be presented on Tuesday 25th November 2008 at BAFTA in London.

To enter, please read the following pdf documents, then complete the entry form below and press submit. Entries will only be accepted if they conform to the information contained in these documents.
Entries must have been broadcast or published in the UK, between 7th June 2007 and 8th June 2008.

Closing date for entries is Friday, 18th July. Please fill in separate forms for each entry.
